DESCRIPTION:

Philippine passport holders may have the surname in their passport changed after marriage, annulment, or death of a spouse. Applicants will be required to apply for a new passport in order for the change of surname to take effect.

REQUIREMENTS:

1. The requirements for changing one’s name in the passport (e.g. from single name to married name) are as follows:

  • If the marriage happened in the Philippines, the applicant needs to present the PSA Marriage Certificate (Security Paper) with DFA Apostille;
  • If widowed, the applicant needs to present the PSA Death and PSA Marriage Certificate (Security Paper) with DFA Apostille;
  • If annulled, the applicant needs to present the annotated PSA Birth and PSA Marriage Certificate (Security Paper), court order (certified true copy), and certificate of finality (certified true copy). All of the documents must have DFA Apostilles.

2. All the requirements listed in applying for a renewal of passport must also be submitted by applicant.
